In all states, it is possible to obtain the record at a DMV office. In general, it is best to pay for the form with a check drawn from a bank, rather than a personal check or cash.

Once you get your driving record, contact Fred Leber (<Frederick.Leber@williams.edu>) to schedule a driving test.

DC: To request a copy of your driving record by mail, send a letter, including your full name, date of birth, Social Security number, driver's license number, photocopy of your ID, and a check or money order for $7, to: DC Department of Motor Vehicles, Attention: Driver Records, 65 K Street, NE, Washington, DC 20001

Delaware: 1. Complete the Personal Information Release Form. 2. 2. Have the form notarized with a seal or stamp (inquire in the Vice President’s Office in Hopkins Hall) 3. Georgia: 1. Maine: 1. If you want the driving record faxed to you, please include your fax number and an additional $2.00 fee.

Massachusetts: 1. Visit the North Adams Office (420 Curran Highway). The fee is $15.00.

Michigan: 1. Print out the form. 2. Mail with $7.00 to: Michigan Department of State Record Lookup Unit, 7064 Crowner Drive, Lansing, MI 48918-1502.

Minnesota: 1. Print and fill out the form. 2. Mail to: Driver and Vehicle Services, Records Unit, 445 Minnesota St., Suite 161, St. Paul, MN 55101-5161

Mississippi: 1. Use the online form.

Missouri: 1. Write to the Department of Revenue, Driver and Vehicle Services Bureau, PO Box 200, Jefferson City, MO 65105-0200. Include the name, date of birth and driver license number of the record you are requesting and $4.00. Make sure you request it sent to either B & G or your SU box.
