In all states, it is possible to obtain the record at a DMV office. In general, it is best to pay for the form with a check drawn from a bank, rather than a personal check or cash.

Delaware: 1. Complete the Personal Information Release Form. 2. 2. Have the form notarized with a seal or stamp (inquire in the Vice Presidentâ€™s Office in Hopkins Hall) 3.
